Pothole & Street Issues at O'farrell St & St Joseph's Ave Anza Vista Sf

That whole section of O'Farrell is littered with potholes. But particularly dangerous are the ones in front of the entrance to the Kaiser ER. One of them has a piece of rebar sicking out (see enclosed). This very dangerous for cyclists. I speak with first hand experience.
